I, the undersigned, acting on behalf of and for the entrepreneur indicated above as an entity applying for membership in the Qualipol Association, declare that the above-mentioned entrepreneur declares their willingness to join the QUALIPOL Association and I request their admission as a supporting member. At the same time, I declare that the delegate of the above-mentioned entrepreneur in the Association will be the person indicated above, and I request that they be accepted as an ordinary member of the QUALIPOL Association.
I also declare that the submission of this declaration is tantamount to the acceptance and adoption of the Association’s Statutes and all applicable regulations and resolutions, which the above-mentioned entity and its delegate undertake to comply with.
Furthermore, I declare that the above-mentioned entity, upon admission to the Association, undertakes to pay the following fees on time: registration fee, membership fees, and all other amounts due to the Association.
